I am inflicted with a schizoaffective disorder which at times causes me to have overwhelming anxiety in my workplace. It has affected my coworkers and certainly my relationship with my boss. I also fear that if I publicly admit it (and take a leave of absence), my career will spiral downward. What are my rights as far as my job goes?

There are legal rights under the American with Disabilities Act that protect workers with mental or emotional problems from employment discrimination. To protect yourself, you must disclose your disability to your boss (who in turn must keep such disclosures confidential). You can also request reasonable accommodations for your illness, as for example, adjustments to your work schedule or physical workspace. If you think you have been discriminated against because of your disability (if you have disclosed your problem), you can file a lawsuit or a claim with the EEOC or with your state's antidiscrimination agency.
